NEW HCFA

• You must convert to the new CMS-HCFA 1500 form beginning October 1, 2006 to no later than 3/31/07.
• Marshall has the forms in stock now as soon as your software company informs you that you can print on the new form.
• If you currently use a RIGHT window envelope for mailing the forms, you can continue to use the same envelope.  However, if you currently use a LEFT window envelope, you will have to switch to a RIGHT window envelope for mailing the new forms.
